A lower body lift is a surgical procedure that removes excess skin and fat from the abdomen, hips, thighs, and buttocks to achieve a more contoured and tightened lower body appearance.

What Is a Lower Body Lift (Belt Lipectomy)?

A lower body lift is a circumferential body contouring proceduremeaning the incision extends all the way around the lower torso, treating the front, sides and back in one operation. The procedure combines the elements of an extended abdominoplasty (front and flanks) with a buttock lift (lower back and upper buttock) and an outer thigh lift, all through the same circumferential incision.


The incision runs just above the pubic hairline at the front, extending around the hips and continuing across the lower back at the level where the lower back and buttocks meet. This placement means the resulting scar can be concealed within the underwear and bikini line in most cases.


The procedure addresses several interconnected problems that develop after massive weight loss:


A lower body lift is distinct from a standard tummy tuck in scope, and distinct from an extended tummy tuck in that it also addresses the posterior — the lower back and buttocks. It is the most comprehensive single-session option for lower body skin excess.

Removes excess skin from the lower and mid-abdomen through a hip-to-hip horizontal incision. Includes muscle repair (diastasis recti plication) and umbilical repositioning. Does not address the flanks, lower back or buttocks. Appropriate where skin laxity is confined to the front of the abdomen.


Extends the horizontal incision around the hips to treat the upper outer flanks and waist. Appropriate where skin laxity spreads from the abdomen into the flanks. Does not address the lower back or buttocks.


Circumferential incision around the entire lower torso. Addresses the abdomen, flanks, waist, outer thighs, lower back and buttocks simultaneously. The most comprehensive option. Appropriate where skin laxity affects the entire lower body circumferencevisible from front, sides and back. Longer operative time and recovery than the tummy tuck options above.


The key clinical indicator is posterior involvement — skin excess visible on the lower back and/or flat, descended buttocks. If a patient has significant skin laxity behind the hips as well as in front, a tummy tuck of any type will not address the posterior adequately. A lower body lift is the appropriate procedure.


Where the skin laxity is predominantly abdominal — with limited posterior involvement — an extended tummy tuck may be sufficient, and the surgeon will advise this at consultation. Where both horizontal and vertical abdominal skin excess is present, a may be more appropriate.

Incision and abdominal work

A horizontal incision is made from hip bone to hip bone just above the pubic hairline, extending around the hips toward the flanks. Excess skin is excised, the umbilicus repositioned, and the incision closed with dissolvable sutures. Drains are placed.


Anaesthesia

Performed as a separate session under TIVA, typically a minimum of six to twelve weeks after Stage 1.


Positioning

The patient begins face-down to allow the surgeon to address the lower back and buttocks. An incision is made above the buttocks across the lower back. Excess skin from the lower back and upper buttock region is excised, tightening and lifting the buttock contour. The incision is closed with dissolvable sutures.


Outer thigh work

The patient is then repositioned to address the outer thighs where skin laxity extends downward. This is carried out through extensions of the same circumferential incision.


Closure and discharge

A compression garment is applied before waking from TIVA. The nursing team monitors you for one to three hours. Written discharge instructions and the 24/7 clinical support number are provided. A responsible adult must take you home — driving is not permitted on the day of surgery.

Recovery After a Lower Body Lift










Recovery from a lower body lift is staged in line with the procedure itself. Because the surgery is performed in two sessions, patients recover from each stage sequentially rather than from one long combined operation.


A slightly flexed posture for one to two weeks is normal as abdominal tension resolves. Most patients take two to three weeks away from desk-based work. Physically demanding roles require four to six weeks. Full exercise at six weeks subject to surgeon review.

What To Expect

Your consultation takes place at our Baker Street clinic with a GMC Specialist Register consultant plastic surgeon and takes approximately 60 minutes. Lower body lift consultations are typically longer than standard cosmetic surgery consultations due to the number of areas involved and the staging discussion. Your surgeon will take a full medical history, review all medications and conduct a thorough physical examination of the entire lower torso — front, flanks, lower back and buttocksassessing the degree and distribution of skin laxity from all angles. They will advise on whether a lower body lift is the appropriate procedure or whether a standard or extended tummy tuck would produce an equivalent result with a shorter incision. Where the lower body lift is appropriate, your surgeon will explain the staged approachabdominoplasty in Stage 1, followed by the buttock and thigh lift in Stage 2 — and discuss the recommended interval between stages (typically a minimum of six to twelve weeks). You are not required to commit to Stage 2 at the first consultation; each stage is consented and quoted separately. Your surgeon will explain the circumferential incision, the expected scar extent and position around the entire lower torso, the procedure steps for each stage, recovery protocols, and all risks and potential complications. High-resolution photographs are taken. A mandatory two-week cooling-off period applies from the date you provide consent. The £100 consultation fee is redeemable against the cost of your procedure. Once the cooling-off period has passed and you decide to proceed, the preoperative assessment team will contact you to complete your medical assessment and provide written preparation instructions. The same preparation applies before each staged session. Stop smoking at least four weeks before surgery and for a minimum of four weeks after. Smoking significantly increases wound breakdown and infection risk along the circumferential incision. This is mandatory for both stages. Stop oestrogen-containing contraceptives or HRT four weeks before surgery to reduce the risk of DVT. Use alternative non-hormonal contraception during this period. Stop aspirin and aspirin-containing medications at least one week before surgery. Stop anti-inflammatory medications such as ibuprofen unless directed otherwise. Avoid herbal supplements for at least one week. Do not eat or drink for six hours before your procedure. Clear fluids — still water, black tea or black coffee — are permitted up to two hours before. For patients who have lost more than 25 kg: maintain a high-protein diet and supplement for at least six weeks before each stage to support wound healing. Post-bariatric patients should ensure nutritional blood tests (protein, B12, iron, vitamin D, zinc) have been reviewed and any deficiencies corrected. Purchase a compression garment before each surgery date — not included in procedure costs. Arrange a responsible adult to take you home and remain with you for the first 24 hours. Prepare a comfortable recovery area at home in advance.
